Your 2024 1095-C will be mailed to your home address in February 2025. If you do not receive your 1095-C by mail, or if you would like an additional copy, you can download your 1095-C by going to HealthEFX. When you get to the site, you will need to register as a new user. If you have any questions, please contact the Pilot Benefits Center at 866-822-6095.

If you are a former team member or do not wish to use your company credentials, click on “Register User” to create a login with MHC. You will need your Social Security Number (XXXXXXXXX). Your User Name will be your email and your password will be what you created during the registration.
